$5,000 Sign On Bonus. ProMedica Hospice, formerly Heartland Hospice, is currently searching for a full-time Social Worker to support hospice patients in their homes in the Rockford and surrounding areas.
The Social Worker is responsible for providing clients and families psychosocial care regarding economic, social, and emotional needs.
This is mainly a weekday position with limited on call.
This position requires a bachelors in social work and certification or licensure in the state of IL and/or WI.

100% travel necessary on a daily basis. Experience in varied multidisciplinary settings in the health care field. Experience working with patients and families coping with terminal illness and death. Excellent communication and interpersonal skills.
