Director of Discharge Management - Full-time / Part-time

The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility's discharge management program in accordance with current federal, state and local standards, as well as established policies and procedures, to assure that the medically related needs of the patient/resident are met on an individual basis.

This position will assess patient needs, develop a continuing care for use at home or a lower level of care after the skilled nursing facility discharge and review the plan with the patient and family. This position plays a key role in the patient's comprehensive rehabilitation.

This position will support our admission and social service departments as needed.

Two years of clinical experience in a healthcare setting in a position that exemplifies patient/family interaction and communication. Problem Solving, Facilitation, Collaboration, Strong Communication, Writing Skills.

Has obtained a certificate and or degree in a program related to the medical field, to include but not limited to, Licensed Nurse, Social Worker, Therapist.
