Kensington Algonquin , an Agemark Senior Living Community is hiring for a Full-Time LPN! For the fifth consecutive year; we are certified #22 in Fortune Best Workplaces for Aging Services 2022! In this Full-Time LPN position, you’ll be responsible for assure the quality, continuity, and appropriateness of care provided by providing direction to wellness staff and communicating with residents, families, and staff to assure quality care.

As a Full-Time LPN, you'll be responsible for oversight of all wellness functions including:

  • Lead by example by interacting with residents and staff every day and walking the building and being involved in care.
  • Review and update service plans according to state requirements.
  • Oversee medication setup and delivery system.
  • Under the RN; will help supervise the performance of individual nursing services as required.
  • Implement wellness programs to maximize health through education, preventative care, etc.
  • Help assess prospective residents to determine care needs and appropriateness for residency. Coordinate Wellness Services procedures to admit new residents.
  • Train/orientate staff concerning wellness services functions and procedures as directed; provide staff in-services as required.
  • Identify at-risk residents and help create a risk management plan if needed.
  • Communicate with residents, families, staff, physicians, and other health care providers regarding resident health issues and service plans.
  • Facilitate alternative placement or supplemental care when appropriate.
  • Respond to resident emergency calls as needed; review and document emergency call responses.

Qualifications of a Full-Time LPN:

  • LPN license in employed State
  • 1 year working with Seniors and working knowledge of health factors associated
  • Knowledge of medication, dosage, terminology, and side effects
  • Proven track record of integrity accuracy and being detail-oriented
  • Excellent verbal/written communication and problem-solving skills
  • Evaluation of the resident's condition for changes or symptoms
  • Maintain the confidentiality of verbal and written information about residents, prospective residents, facility operations, and personnel
