Summary

Part Time ED Care Coordinator - RN - Utilization Review - Remote Position 

Position Summary:

The Emergency Department Care Coordinator-Utilization Review RN, in collaboration with the clinical team to include the medical provider, delivers navigation and coordination of services and interventions for identified patient's presenting to the Emergency Department. The Care Coordinator provides status recommendations, strives to promote patient wellness, improved care outcomes, and efficient utilization of health services. Ideally the functions of the Care Coordinator will impact metrics to include throughput, length of stay, readmissions, potentially avoidable utilization (PAUs) and denials. The Care Coordinator serves as the Subject Matter Expert for Medicare reimbursement compliance. The position does require strong critical care and/or emergency medicine clinical knowledge.

 

Essential Functions:

 

Assessment Performs initial Care Management assessment to determine, based on patient's condition and presentation, care coordination and discharge planning needs as appropriate in the Emergency Department.

Tasks: Focusing on patients where decision to admit has not yet been made, performs an initial care management assessment or review to determine the anticipated status and plan of care. Identifies patients that are 30 day readmissions, 30 day revisits and high utilizers and directs them to the appropriate ED resource, such as navigation, social work, community care coordination, or other available programs. Performs reviews and applies standardized and approved criteria for medical necessity and level of care and documents its application per departmental policy. Collaborates with physicians, nursing, bed management and interdisciplinary team to determine appropriate level of care prior to bed placement. Reassesses admitted patients boarding in the emergency department while awaiting inpatient bed placement for continued level of care. Provides feedback to Emergency Department Physician, Attending Physician, Nursing, affected personnel/departments with regard to level of care decisions. Escalates cases, as appropriate, to Physician Advisor. Identifies patients that are out of network (e.g., Kaiser patients) and, with Emergency Department Physician approval, facilitates transport to appropriate facility. Dialogues with payor to determine need for relocation to in-network facility.

Planning Creates a focused, anticipated discharge plan of care for identified patients.

Tasks: Works with Emergency Department charge nurse and physicians to prioritize patients for Care Coordination intervention. Collaborates with Physician and Care Providers to align services for patients who do not want aggressive medical treatment i.e. NO CODE; hospice Seeks plans and strategies, in collaboration with the healthcare team, to minimize frequency of admissions or Emergency Department visits.

 

Qualifications/Requirements

 

Education Requirements:  

Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field

 

Education Discipline:

Associate Degree in Nursing required, Bachelor of Nursing preferred. Requires previous experience in direct patient care in critical care or emergency department setting. Utilization Review experience
