Job Description and Responsibilities

Under the direction of the Director of Assisted Living and Care Management, the Licensed Practical Nurse is responsible for providing for the health care needs of Duncaster residents in the apartments. Functions as a professional caregiver who is accountable for implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs. Acts as the charge nurse and works to resolve issues and concerns of CNAs. Performs all job responsibilities according to Duncaster's Policies & Procedures.

  • Carries out the plan of care for the resident and instruct the assisted living aide of services to be provided.
  • Complete data collection as necessary based on the resident's condition.
  • Prompt response to changes in resident condition to include communication with physician or practitioner and family or responsible party as indicated.
  • Referral to appropriate professionals or agencies whenever the condition necessitates ensuring that if the resident's condition is no longer chronic &stable, services of a licensed home care agency are engaged, or other appropriate arrangements are made.
  • Manage medications to include verifying orders, pre-pouring or working with pharmacy managing pre-pour, administering and documenting; communicate with MD or practitioner when indicated.
  • Provide treatments as prescribed; effectively communicate to MD or practitioner when indicated.
  • Respond to calls for aid to include evaluation for injury, completion of documentation and communication with appropriate parties, and adjustment to plan of care if indicated.
  • Accurate and timely regular documentation on 24-hour report and resident chart of interventions, changes in condition, referrals to other services, notifications of MD and/or family, etc.
  • Utilizes standard precautions at all times and reports potential exposure or hazardous condition to supervisor.
  • Responsible for electronic medical record charting.
  • Supervises provision of care by Assisted Living aide.
  • Demonstrate skills specific to providing care to residents affected by dementia.
  • Assume administrative responsibilities within the department to include chart audits and other duties as requested by the RN Designee and Director of Assisted Living.
  • Communicates with Dogwood aide staff each shift and as needed.
  • Audits charts along with other Quality initiatives designated by the Director of Assisted Living and Care Management.

Education and Experience

  • Must hold a valid LPN license in the state of Connecticut with a minimum of two years of full-time clinical experience in nursing.
  • Must have the ability to read, write and speak English.
  • Must have the ability to effectively communicate in both oral and written form.
  • Must be able to perform the essential job functions with or without reasonable accommodation.
  • Comprehensive knowledge of geriatric nursing care, preferable in outpatient services.
  • Comprehensive knowledge of principles of nursing, and physical evaluation skills.
  • Strong organizational and administrative skills.
  • Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and ALSA staff.


Hours

1st: 7:00am-3:30pm

2nd: 3:00pm - 11:30pm

3rd: 11:00pm - 7:30am

Partial Shifts

7:00am-10:00am or 8:00am-11:00am


Pay Rate

$30.00 p/h + shift differentials ($2.00 p/h weekday evenings, nights & weekend days, $3.00 p/h weekend evenings and weekend nights)
