    Overview: 

    The Integrated Care Service Coordinator assists the Supervising Integration Specialists (Registered Nurses)and provides a central point of contact for all Case Management and Behavioral Health Home (Integrated Care Management) services. 

    Type: Hourly, Full-time 
    Location: Duluth, MN 
    Pay:$21(with competency/outcome-based increase opportunities) 

    Responsibilities: 

    • Contacting physicians and other service providers to obtain medical records as needed 

    • Uploading received medical records and recording diagnoses, physician orders, medications, lab results, and other pertinent information in each person's file, and transcribing pertinent information to the Electronic Health Record  and informing providers 

    • Taking calls from recipients as needed when they are unable to reach their Integrated Care Managers staff, solving their problems and connecting them with necessary supports 

    • Communicating with Managed Care Organizations for all persons referred to the BHH service to verify eligibility, payment arrangements, and coordinated care 

    • Conducting regular Patient Registry Reports targeting specific health care conditions and populations as directed by the Integration Specialist for triage, prioritization and quality assurance. 

    • Compiling regular reports on health care outcomes and costs for individuals served in the program. 

    • Appointment  and meeting scheduling 

    • Training new Integrated Care Managers on the use of the Electronic Health Record 

    Qualifications:  

    • Proficiency with a variety of technologies, including word processing, spreadsheets, and electronic health records, and other data analytics tools.  

    • The ability to work quickly and efficiently in a fast-moving and growing program.  

      And one of the following:

    • A Community Health Worker 

    • A Peer Support or Family Peer Support Specialist 

    • A Case Manager or Case Management Associate 

    • A Mental Health Rehabilitation Practitioner or Worker 

    • A Community Paramedic 

    • A Certified Health Education Specialist 

    Employees must pass a criminal background and MVR check, maintain a valid driver's license and vehicle insurance.

    Preferred Qualifications: 

    Familiarity with reading interpreting medical records, HIPAA, and data privacy laws.
