The Hospice Nurse Case Manager will be responsible for coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families. The ideal candidate for RN Case Manager for Hospice will collaborate with the patient and / or primary caregiver(s) and the other members of the Hospice Interdisciplinary group (IDG) to develop and implement the plan of care (POC) for the patient.
The Hospice Nurse Case Manager should have all basic nursing skills, including but not limited to nursing assessment, current symptom management knowledge, patient / caregiver teaching and documentation along with advanced skills such as collaboration, communication, negotiation, mediation, anticipation and planning. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of the organization.
