We have a full-time opening for a Case Manager/Social Worker.

Must be able to work various shifts per week.

• Must have 2 or more years experience.
• Must have a high school diploma or equivalent.
• Be authorized to work in the United States.
• Must have reliable transportation.
• Background check required.

Wage: $37,000

Full-time; Monday-Friday 10-6 pm. Salary 37,000. Must have a bachelors degree in Social Work, Psychology, Education ect.  The Case Manager will be responsible for communication and coordination between the S.T.E.P. program, the home, the Service Agencies and act as liaison between United Cerebral Palsy and the student's home concerning all social/emotional, behavioral, emotional, and health issues. Position will also be responsible for Case Management records relating to each student. Attend workshops and read current literature to keep abreast of social services, behavior modification, family support, service intervention information, etc. Meet with the S.T.E.P Coordinator on a regular basis to discuss the program, the students, services concerns, problems, etc. Must have experience working with students with disabilities. Must have strong writing skills and a good use of computer technology. Must have a vehicle as driving to the different sites is required
