The Everett Internal Medicine department is seeking a LPN Health Coach to join their team part full time in Everett. Schedule is Monday-Friday 8-5

The LPN functions as an integral member of the team and is responsible for helping patients in health/life style management, prevention and risk reduction to improve their overall health. This role is responsible for engaging, educating, and empowering patients to take an active role in their health. As part of the Integrated Care Team provides patients with the tools needed to make health lifestyle choices. They will help coordinate necessary services within and outside of TEC.
The LPN is responsible for and not limited to:
Preferred Experience and Credentials:
Education: Graduate of an accredited school of nursing.
Experience: One year of professional nursing experience in a clinic setting. Internal Medicine or geriatric experienced preferred. Care coordination experience or skilled nursing home experience.
Certificate/License: Current WA State Practical Nursing license required. Current BLS certification required.
