POSITION SUMMARY: The Social Services Coordinator is responsible for the coordination of supportive services for residents in order to help them maintain their independence and age in place. Reports to Director of Resident Services.
DUTIES & RESPONSIBILITIES:
Assists in identifying residents who may need assistance. Completes psychosocial assessments, as appropriate. Encourages residents to be proactive in meeting their social, psychological and physical needs.
Advises residents and communicates with families regardingservices that may be available to help residents maintain a self-reliant lifestyle.
Educates residents on application procedures for community services. Assists with application procedures, when necessary, but avoids creating unhealthy dependence. Advocates on behalf of residents for adequate, timely and cost-effective provision of services.
Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory. Acts as a liaison between service providers and residents, when appropriate.
Provides limited and interim case management for residents when services are not being provided by a community agency or while awaiting the implementation of services. This may include a comprehensive, psycho-social assessment and development of a service plan.
Communicates with healthcare providers and family members, when a resident ishospitalized, to determine if services are needed to return safely to an independent living setting.
Assesses if residents living independently may need additional services and may benefit from a high level of care facility.
Processes referrals from Administrative staff. Meets with residents in order to discuss and identify problems/issues and takes necessary actions to assist residents with resolution of issues. Communicates to Administrator on disposition of staff-initiated referrals, as appropriate and within the bounds of confidentiality.
Assists in determining when it is appropriate to address disruptive behavior and/or disputes among residents in conjunction with Administrator, intervenes when necessary.
As needed, may assist residents in understanding lease and tenancy obligations after initial referral to Administrative staff.
Arranges and promotes Wellness Programs for residents. Programs may include health screenings and educational lectures. Schedules educational speakers for monthly Community Forum Lunch.
Documents contacts with residents, providers and families. Maintains individual files and creates timelines on resident issues in conjunction with the Administrator. Keeps files secured to insure confidentiality.
Researches and becomes familiar with community resources that could provide supportive services to the residents.
Assists residents in building informal support networks among themselves and with family members as appropriate.
Assists Independent Living Administrator with potential new resident interviews, offering information regarding the role of the Social Services Coordinator.
Meets regularly with the Director of Resident Services for resident review; seeks assistance as needed with problematic situations and coordination of team conferences as appropriate.
Completes the HUD Semi-Annual report additional reports and timelines as required.
Completes the annual training requirements as required by HUD, and attends LSC quarterly trainings.
Reports all suspected abuse situations to Adult Protective Services, as mandated, communicates with the Director of Resident Services when/if reporting occurs.
Adheres to the American Association of Service Coordinators "Code of Ethics" and Lesley Senior Communities' "Policy Manual" and "Social Services Procedures".
Other duties as assigned.
Bachelor's degree required in related field, Master's degree preferred.
2 years senior services experience.
Familiar with and committed to the needs and concerns of an elderly population.
Excellentcommunication skills: written/documentation, problem solving, organizational and advocacy skills. Case Management experience helpful.
Ability to interact in a caring, sensitive and responsive manner with residents while maintaining professional boundaries.
Awareness of behavioral, mental health issues.
Ability to exercise discretion, independent judgment and effective problem solving techniques.
Working knowledge of community services, federal and state entitlement programs or a commitment to become familiar with such services and programs.
Alert to legal liability issues related to providing service coordination. Ability to work well under stress.
Knowledge of Windows and Microsoft office software.
Ability to talk and hear; visual acuity such that (s)he can use a computer, read, and write reports.
Working conditions are that of an office environment. Primarily sedentary work.
