        Job Description

        Fellowship Health Resources affiliated with Elwyn in 2018 and provides behavioral health services to improve the quality of life for individuals living with mental illness and addictions

        To provide leadership, individual family and group counseling in therapeutic setting designed to serve people of all ages

        Pre-employment Education/ Experience

        Masters Degree in Clinical Psychology, Education al Counseling, Rehabilitative Counseling, Social Work, Nursing or closely related discipline. A minimum of two years experience providing counseling services in a Behavioral Health setting. Possess a license such as an LCSW, LICSW, LCP, LMHC, LCPP Experience with Addiction treatment desired. Knowledge of Diagnostic & Statistical Manual (DSM) of Mental Disorders and its clinical application; knowledge and practice evidence based clinical interventions (i.e., Motivational Interviewing, CBT, and DBT)

        Region Specific Criteria

        MA CBFS model requires a Licensed practitioner of the Healing Arts (LPHA) and could include a registered nurse with the appropriate level of experience.

        Co-Worker Relations / Teamwork. Working effectively and cooperatively with co-workers (e.g., peers, management).         • Screening, Assessment, And Individual Action Planning:
        • III. ESSENTIAL FUNCTIONS AND PERFORMANCE STANDARDS:
        • Screens, assesses, and develops individual action plans collaboratively with individuals, utilizing a person-centered, and recovery based framework in compliance with FHR, CARF, State/County requirements. Ensures the appropriate needs assessment, information review, medical/physical history and other pertinent information is obtained reviewed and evaluated
        • Makes clinically sound observations and interventions in a timely manner. Ensuring the consumer's health and safety

        Therapeutic Support

        • Develops and maintains therapeutic relationships with persons served as well as maintain a positive relationship with external community resources.
        • Integrates the Prism Model for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention .
        • Practices a trauma informed approach to treatment planning and service delivery.
        • Provides face-to-face crisis intervention, providing short-term intervention to decrease risk, manage symptoms, and maintain safety.
        • Ensures that staff and individuals are provided with information related to the use of recovery tools, techniques, and self management strategies.
        • Offers family support, education, resources, and training related to the treatment and effects of mental illness and Addiction disorders.
        • Develops, implements, and facilitates clinical groups based on the needs of the individuals enrolled

        Communication & Teamwork

        • Ability to Express thoughts, ideas, and opinions clearly, fluently, and concisely.
        • Ensures that information is passed on to others who need to be kept informed.
        • Listens attentively to others and responds appropriately to questions and comments.
        • Ability to Ask appropriate questions in order to obtain information and to determine the needs of others.
        • Ability to Comprehend and follow oral instructions.
        • Leadership, Coaching, & Professional Development
        • Provides clinical supervision to staff.
        • Provides clinical leadership and informed opinion regarding clinical decision-making, implementation of services, and outcomes.
        • Participates in regional or organizational performance improvement initiates
        • Conducts training and/or educates staff/persons served in mental health recovery, managing symptoms and stressors regarding psychotropic medications, treatment planning, skills training regarding co-occurring disorders such as Addiction disorders and the knowledge required to help persons served transition to independent living.
        • Coaches staff in to provide services in a holistic manner which includes but is not limited to behavioral, medical and social factors.
        • Serves as a member of a multidisciplinary tea m
        • Demonstrates strong leadership and clinical skills to promote an effective team environment.

        Documentation

        • Produces clinical documentation/records that is a clear and concise reflection of the persons served Strengths, Needs, Abilities and Preferences.
        • All documentation is to be in accordance with the guidelines of State/County policies and procedures, FHR policies and procedures, and CARF standards.
        • Maintain and or produces records, report & statistics necessary for program functioning and evaluation as required by FHR/QI Department, or Department of Mental Health.
        • Documentation in chart is exclusively done from the perspective of a licensed clinician.Region Specific CriteriaMA- Performs Licensed Clinician as LPHA responsibilities in compliance with FHR, CARF, Massachusetts DMH State/County requirements,
        • Other duties as assigned.

        Working Conditions

        Necessary Travel:

        • Valid driver's license required. Provides services to individuals in the community Daily transportation of individuals and as needed for training purposes
        • Driving in inclement weather as needed.

        Unusual Work Hours:

        • Hours as assigned by Director. Scheduled holiday.

        Unusual Environmental Conditions:

        • Work area - inside and outside,.. Environment may be noisy at times. Hazards - potential for exposure to infectious disease. Physical / Mental Requirements:
