General Summary
Responsible for the delivery of program service support and referrals through the Comprehensive Energy Assistance Program (CEAP), and related Programs. Coordinates access to needed services, materials, programs and information for all client families.
Primary Responsibilities
1.Performs in-take and assures that all documentation is provided for applications for assistance for the CEAP, Neighbor-to-Neighbor, WAP and related programs.
2.Requests energy consumption reports from energy vendors.
3.Responsible for maintaining files, and payment history reports.
4.Responsible for documenting family progress by preparing, updating and maintaining progress notes and goals on the Activities and Contact Record.
5.Provides information and referral; assists families in gaining access to needed medical, social, educational, developmental and other appropriate services, including transportation.
6.Responsible for the entry of client data and the use of the data for CEAP, WAP, and related assistance programs.
7.Non-physical demands include performing multiple tasks simultaneously, the ability to meet multiple deadlines, judgement decisions and working closely with others as part of a team.
8.Must be willing to travel as necessary.
9.Other duties as assigned.
