Provides skilled nursing services and assists with the coordination of home health care services for assigned patients.  Ensures quality and safe delivery of home health care services in compliance with all applicable laws, rules, regulations and policies.

Position is  guaranteed 40 hours weekly at an excellent hourly rate.  Hourly rates cover your drive and documentation times.  Mileage is paid at 56 cents per mile.  Full benefits within 30 days.  Annual bonus potential, annual reviews, PTO, paid holidays.  Opportunities to continue to progress in your career!

Primary Responsibilities:

• Provides skilled nursing services that require substantial and specialized nursing care including medication administration, wound care, wound vacs, IV administration including PICC and peripheral lines, therapeutic treatments, and skilled teaching as ordered and indicated


• Plans and implements patient health teaching and health counseling regarding the disease process(es), self-care techniques and prevention and involves the patient's significant others in accomplishing health goals while promoting patient / family independence


• Participates in health promotion and disease prevention activities


• Initiates appropriate preventive and rehabilitative nursing procedures following the physician's plan of care


• Assesses the patient's condition during every home health care visit; insures assessments are communicated to the Clinical Team Manager on a daily basis; nursing interventions are implemented to meet patient needs and changed conditions


• Performs care management duties on patients as assigned including: notifying the physician of changes in the patient's condition or progress toward goals, obtaining needed physicians orders, reassessing the patient for recertification and coordination of care by reporting of significant findings to others on the healthcare team Documents all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notes


• Expresses verbal and written communication in a clear, positive and cooperative manner

Required Qualifications:

• Accredited high school/GED


• Current and unrestricted Texas LVN license or compact state LVN license


• 1+ years home health care or related environment


• BLS and/or CPR certification


• Developed computer skills


• Ability to lift and transfer patients


• Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area


• This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease


• You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.


• Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained
