As a Case Manager you will deal with all client and CAREGiver issues over the phone. Connecting with families, healthcare partners and others to develop solutions to meet the needs of our Clients. The responsibility of this job includes:
&bull Reviewing Assessments, Plan of Care and overall Care Goals
&bull Establish a plan of care that meets care plan needs
&bull Ensure the client and their family's expectations are met and quality care is delivered
&bull Communicate and work through delicate conversations and situations
The Case Manager needs to possess:
&bull Case Management experience preferred
&bull Excellent communication skills
&bull Ability to have direct conversations
&bull Ability to provide solutions
&bull Must be organized
&bull Ability to complete high volume of paperwork with accuracy
&bull Must be willing to travel throughout Home Instead's local territory
&bull Participate in on-call rotation
