    Job Description

    UnityPoint Health - Fort Dodge

    Unit Clerk - Acute Care

    FTE: 1.0 (Full-time, 40hours per week)

    Shift: 6:30AM - 3:00PM

    • Greet patients and families in cheerful and welcoming manner.
    • Display friendly, courteous behavior to patients, visitors, and other hospital personnel.
    • Answer all incoming phone calls in friendly and enthusiastic voice.
    • Screen and direct phone calls to proper location.
    • Take phone messages accurately and responsibly.
    • Page appropriate staff.
    • Monitors scheduled patients to insure correct/signed physician order, insurance pre-authorization, medical necessity and Advance Beneficiary Notice (ABN)'s.
    • Process modality schedules and paperwork for following day.
    • Check for current medication list for all patients.
    • Check for appropriate history/symptoms and ICD codes.
    • Verify patient exam history.
    • Verify need for ABN and prepare form.
    • Add questionnaires and consents to patient paperwork.
    • Check and print insurance pre-authorization forms.
    • Highlight important information on scheduling worksheets.
    • Order patient exam in electronic medical record (EMR)
    • Inform patients of exam status, including any delays.
    • Answer patient questions or concerns or direct patient to correct staff who can provide accurate information.
    • Review registration information on face sheet, make changes if necessary.
    • Check for placement of patient identification bracelet on patient.
    • Assist patients in completing exam worksheets, consents, ABNs.
    • Check for prior exams.
    • Purge jacket of films according to department policy.
    • Notify technologist of patient arrival.
    • Process ABN after patient has signed.
    • Sort and file processed exam requests.
    • Sort and process patient exam paperwork.
    • Send completed paperwork to Health Information Management (H.I.M).
    • Process completed Calcium Scoring results to patient and referring physician.
    • Tracking of all incoming and outgoing films and CDs.
    • File and maintain all requests for images.
    • Orders and maintains necessary supplies according to department needs.
    • Operates department paging system and or cell phone
    • Communicates with other members of the healthcare team to coordinate other patient services.
    • Demonstrates positive customer relationships.
    • Adheres to hospital and department dress code.
    • Performs duties following unit and hospital standards.
    • Performs duties necessary to allow for efficient patient processing.
    • Proficient use of multiple computer software applications.
      • EMR
        • Order patient exam
        • Order Lab work.
        • Look up results (i.e. lab work).
        • Patient demographics for insurance and guarantor information.
        • Print patient labels.
        • Print demographic sheets.
      • Sunquest
        • Assure patient exam order crosses interface from EMR
        • Make changes to patient location and account number.
        • Access to reports and faxing reports to physicians upon request.
        • Assist radiologist billing by placing file copy on system.
      • Smart Term
        • Run OAR daily.
        • Run HVSRP daily.
        • Separate and place in monthly binder.
      • Caremedics
        • Check LMRP/ICD codes.
        • Prepare ABN, if needed.
      • PACS Scanner
        • Scan outside reports to current study.
        • Scan questionnaires for current study.
      • PACS Cube
        • Complete worksheet.
        • Enter patient information.
        • Choose appropriate images and burn to CD.
        • Prepare Fed-Ex information and package CD for pick up.
        • File CD in appropriate area for delivery.
      • PET/CT Burner
        • Complete worksheet.
        • Enter patient information.
        • Properly identify CD with patient's name.
        • Prepare CD for delivery.

    Minimum Requirements

    High school graduate or equivalent


    Office clerical experience preferred.


    Valid driver's license when driving any vehicle for work-related reasons.


    Demonstrates effective written and oral communication skills.

    General clerical skills.

    Basic computer skills.


    Lead body aprons, thyroid shields, lead gloves, and lead eyewear are in the department.
