    Job Description:
    • Provide skilled nursing assessment and implement patient care as directed by the attending physician
    • Obtain data on physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team
    • Perform assigned nursing procedures, including the administration of treatments and medications in the care of the patient, in accordance with the State Nurse Practice Act and the plan of care
    • Notify case manager, physicians, interdisciplinary team and families of changes in the patient's condition
    • Understanding of Home Healthcare philosophy and ability to assess and manage symptoms
    • Provide effective teaching to family, caregivers and interdisciplinary team members to ensure continuity of care for the patient
    • Develop and maintain familiarity with Federal, State and Company policies and procedures
    • Supervise and assess the delivery of care by the Home Health Aide and the Licensed Practical Nurse as needed
    • Share in providing 24 hour, 7 day per week coverage to patients and families as necessary
    • Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency
    • Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
    • Maintain a high degree of confidentiality at all times due to access to sensitive information
    • Abide by all regulations, policies, procedures and standards
    • Follow all Medicare, Medicaid, and HIPAA regulations and requirements
    • Performs other duties as assigned
