The Institute for Family Health is seeking compassionate, dedicated, and dependable candidates for the position of Community Health Worker (CHW). A CHW identifies and works with any women of childbearing age to overcome social or health related barriers with the ultimate goal of promoting healthy pregnancies, maternal and infant health.
Have direct interaction with the community at large as an ambassador for women's health
Provide insight and resources to assigned patients
Carry a caseload of women and/or families
Make referrals to resources in the community to overcome barriers (rather than act as a standard case manager)
Complete the proper paperwork, documentation, and follow up
Attend trainings and community events
Conduct interviews in the home and other locations to identify areas of need and assist individuals and families in accessing health, social services, and other services needed to assure positive health outcomes
H.S. Diploma required. Bachelor's Degree preferred
Prior experience with at-risk families in health care or human services setting preferred
Bilingual Spanish/English preferred
Computer skills, Microsoft Office, data entry, PowerPoint, advanced computer skills for tracking and organizing information
Shows respect and sensitivity for the needs and rights of others, including those with differing cultural, racial, sexual, ethnic, and gender identities
