POSITION SUMMARY
The Admission Coordinator will be responsible for facilitating the process of patients moving into a skilled nursing facility. This includes communication with hospitals and discharge planners, some marketing, insurance authorizations, communication with patients and their families and extensive paperwork. Must be able to work in a fast paced environment and to maintain good relationships with nursing staff.

ESSENTIAL DUTIES:

Customer Service

  • Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  •  Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
    • Identify yourself in a pleasant and positive manner.
    • Take responsibility for helping the caller.
  • Maintain positive relationship with hospitals and communicate census.
  • Facilitate re-admission of residents from the hospital.
  • Field inquires – tours provide information about facility, maintain waiting list.
  • Complete admission paperwork with patient and/or designated representative.  Ensure all required documentation is obtained.
  • Assure that the patient is aware of Resident’s Right and other services offered such as dental, podiatry, optometry and hearing testing.  Assist them in exercising their rights and services as desired.
  • This position requires the use of a cell phone to conduct essential job duties and is not subject to the “no cell phone” policy.  Cell phone usage is to be confined to business calls during working hours and a company paid cell phone must be used so as not to incur extra expense for the company.  This position may also require the use of a camera.

EDUCATION:

Higher education (Associates or higher degree preferred).  Computer literate.  Knowledge of and experience with various filing systems, typing and proofreading. Prefer candidate with a minimum of one-year experience working hand on with senior population.

EXPERIENCE:

  • Previous experience in patient care setting preferred.
  • Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.

PHSICAL DEMANDS:

  • To be able to lift up to 20 lbs.
  • To be able to reach, bend, kneel and squat.
  • To be able to walk and stand on a repetitive and constant basis.
  • To be able to use step stools.
  • To be able to hear call system, pager signals and spoken English language.
  • To be able to wear protective gloves supplied by the facility.
  • To be able to be in contact with domestic pets.
