We are seeking a knowledgeable, compassionate RN to handle patient visits with our Hospice patients. As a Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.

 

 

Please note: This is a full time case manager position and will require travel throughout our service area to conduct visits at our patient's home, nursing home or other facility.

    

Responsibilities for the Hospice RN Case Manager include:


  • Conducting routine visits at home or skilled nursing facilities

  • Coordinating, assessing, and implementing patients' care

  • Providing accurate and timely documentation of patient services to reflect plans of care

  • Admissions and recertification visits

  • Attending weekly interdisciplinary meetings

  • Interacting with other group members such as CNA's, LPN's, social workers and dieticians

  • Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials

  • Participating in on call as needed (which may include evenings, weekends and holidays)

Requirements

Qualifications:


  • RN licensure (current, unrestricted) in the state location of position

  • 3 years of clinical experience is required

  • Reliable transportation including a valid driver's license and auto insurance

 

Additional Assets Preferred:


  • Excellent communication skills

  • Experience in long-term care is a plus

  • Background in home care and/or hospice care

  • Bilingual/Multilingual
