As a Palliative Care Nurse Practitioner you will work in collaboration with the Medical Director and other interdisciplinary team members to deliver direct palliative care and related health care services to patients and families within our program.

 

Please note: This is a per diem/as needed position. The Palliative Care NP will respond to requests for Palliative Consultations in either facility or community within 24 hours of the referral and complete Optum Palliative Assessment as well as communicate consultation outcome with patient/family as well as palliative consult referral source.

 

Responsibilities of the Palliative Care Nurse Practitioner may include but are not limited to the following:


  • Performing bedside palliative care for patients

  • Conducting a patient/family evaluation

  • Facilitating highly effective conversations regarding goals of care with patients, families, interdisciplinary team members, and other providers

  • Performing highly effective pain and symptom management treatment and care plan recommendation with Medical Director, families, and other providers to enhance patients' quality of life

  • Participating with others to deliver plan for optimal coordination with primary physicians and other treating providers including the IPA and local care delivery system

  • Providing community, patient, and family education regarding our palliative care program

  • Consulting with palliative care physician and teammates as needed

  • Communicating and consulting with the patient's primary physician regarding services provided and collaborating with other physicians as needed to provide optimal care

  • Participating in continuous quality improvement and ongoing program development and innovation

  • Preparing and maintaining accurate patient records, charts, and documents to support sound medical practice (both paper and EMR)

  • Adhering to the practice of confidentiality regarding patients, families, staff and the organization

  • Availability to travel throughout the assigned territory in order to conduct visits and initial assessments.

 


 


 

Requirements

Required Qualifications:


  • Current and valid license to practice in the State of the program as a Nurse Practitioner with furnishing privileges

  • Minimum 1 year hands-on Nurse Practitioner experience

  • Experience with adults facing complex and advanced illnesses

  • Certified or working toward national certification as a Nurse Practitioner (Adult/Family/Geriatric/Palliative Care)

  • Current and valid Driver's License, including reliable transportation and automobile insurance

  • Active DEA license or ability to obtain

  • Prescriptive Authority or ability to obtain

  • NPI (National Provider Identifier)

  • Strong verbal and written communication skills

  • Ability to work independently and also as a team

 

Preferred Qualifications:


  • Palliative care and/or Hospice experience

  • Clinical nursing experience in oncology, geriatrics, ICU and/or CCU

  • Experience in education and counseling

  • EMR (electronic medical records) experience
