About this job

Responsible for providing assessment of social and emotional problems that are, or are expected to be an impediment to the effective treatment of the patient's medical condition or their rate of recovery. Develops and implements a social work plan of care.Collaborates with professional staff on psychosocial problems and appropriate interventions. Provides patients and families with information on community resource options and assists in linking with needed resources.

Masters Degree in Social Work required with current NJ LSW
1+ year of supervised clinical experience; community health experience preferred
Excellent verbal and written communication skills
Excellent interpersonal skills
Ability to work with multiethnic and diverse socioeconomic populations
