As a part of our continued growth we are searching for a new RN Complex Case Manager to join our team...
Duties of the Complex Case Manager include:
- Takes in-bound calls and places out-bound calls as dictated by consumer and business needs
- Provides health education and coaches consumers on treatment alternatives to assist them in making informed decisions about healthcare choices
- Supports consumers in selection of best physician and facility to maximize access, quality, and to manage healthcare costs
- Coordinates services and referrals to health programs
- Prepares individuals for physician visits and assesses and triages immediate health concerns
- Manages utilization through patient education
- Identifies problems or gaps in care offering opportunity for intervention
- Assists members in sorting through their benefits and making choices
- Visiting local area Medical Offices on a monthly basis
Requirements
Basic Qualifications:
- Current and unrestricted RN license in California
- 2 or more years of Case Management experience: Complex cases, Disease Management or Special Needs Patients
- Excellent written and verbal communication skills
- Proficiency in Microsoft Office: Outlook, Word, Excel & PowerPoint
Preferred Assets:
- Certified Case Manager (CCM)
- Managed Care work experience
- Previous Telephonic Case Management experience
- Bilingual in Spanish
- BSN degree
