Total Home Care Pharmacy is part of Southwest Medical Associates (SMA).  Southwest Medical Associates is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 200 providers, fourteen clinical locations including nine health care centers, five urgent care clinics and an outpatient surgery center. In this position you will provide pharmacy consultation to a contracted facility and its medical and nursing staff.  You will work within acute, subacute, or long term care facilities. You will review charts and monitor the patient's therapy and progress or  potential outcome. You will work closely with the patient's physician and facility nursing staff to optimize the safety and efficacy of the patient's medication regimen. You will enjoy a high degree of independence and autonomy.

 


  • Review medication orders, check for appropriateness of therapy and recommend changes in antibiotics, total parenteral nutrition, pain management, anticoagulation

  • Participate in quality improvement programs, productivity projects, resource  projects

  • Intervene in complicated cases where pharmaceutical expertise is required to improve quality of patient care or when appropriateness of therapy is questioned.

  • Communicate with Total Home Care pharmacy, nursing, and physicians to resolve and clarify discrepancies and medication orders throughout the various facilities.

  • Follow automatic substitution policies, implement formulary, and place stop dates on antibiotic or anticoagulation therapy where appropriate to lower Total Home Care Pharmacy's acquisition costs.

  • Exercise positive customer service and communication skills with  contracted facility and THC Pharmacy and with other Total Home Care and Family Companies staff.

  • Represent Total Home care at all required committee meetings at contracted facilities and within Family Companies.

Requirements

Required Qualifications: 


  • Bachelor Degree in Pharmacy

  • Active, unrestricted license to practice as a Pharmacist in the State of Nevada

  • 2+ years experience in clinical pharmacy

  • Long Term Care or retail experience preferred

  • Experience in IV admixture programs, nutritional support including but not limited to the writing of TPN orders, antibiotic therapy including but not limited to aminoglycoside dosing, clinical monitoring, and  renal dosing.

  • Experience communicating complicated medical information in both written and verbaly. 

  • Doctor of Pharmacy strongly preferred
