As a Case Manager in our Atlanta (Dunwoody) office, you will review patient status over the phone while ensuring they are receiving the best care. We are currently hiring for evening shifts (Noon-9 or 12:30 - 9pm, Monday through Friday plus Saturday or Sunday daytime rotations every other week.  We also offer the ability to work 4-10 hour shifts, which will include 3 week days and 1 weekend day each week. 


 


 


Primary responsibilities include:




  • Making outbound calls to assess members' current health status


  • Identifying gaps or barriers in treatment plans


  • Providing patient education to assist with self management


  • Interacting with Medical Directors on challenging cases


  • Coordinating care for members


  • Making referrals to outside sources


  • Coordinating services as needed (home health, DME, etc)


  • Educating members on disease processes


  • Encouraging members to make healthy lifestyle changes


  • Documenting and tracking findings


  • Making post-discharge calls to ensure that discharged member receive the necessary services and resources

Requirements


Requirements:



  • A current, unrestricted RN license in the State of Georgia

  • 3+ years experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Case Manager for another insurance company; at least one year of clinical experience must be dealing with adult patients

  • Computer proficiency, to include strong data entry skills and the ability to navigate a Windows environment

 


Additional assets include:



  • BSN

  • A background in managed care

  • Case or disease management experience

  • Experience/exposure with discharge planning

  • Bilingual (Spanish/English) skills

  • Medical/Surgical, Home Health, Diabetes or Cardiac experience is strongly preferred (certification in one of those specialties is a plus)
