Description

Every person needs the support of others, but we look for very special people to support individuals with disabilities;behavioral or medical complexities; and adults with brain injury. As a Registered Nurse (RN) with Illinois MENTOR Community Services you’ll be one of those special people.

Illinois MENTOR Community Services is seeking a Registered Nurse to coordinate the development and implementation of services and treatments for adults with intellectual and developmental disabilities and other complex needs for programs in Matteson and the surrounding area. This position will provide nursing services, training and support, oversee all services provided, monitor quality of care delivered and ensure regulatory and corporate program compliance. The position is approximately 20 - 25 hours per week and will require routine travel with in the designated area.

Responsibilities:

  • Maintain current records for all independent contractors on caseload including training, monthly and annual mentor contract compliance forms, respite and support plans and tracking of respite support workers
  • Assist with independent contractor in-services, planning and skill development.
  • Maintain current and complete client records and reports in accordance with corporate and regulatory standards
  • Conducts medical assessments and participates in the development and implementation of service plans.
  • Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan.
  • Provides and documents medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family. Presents medical skills components at Pre-service Orientation.
  • Maintains contact with primary care physicians and specialists, and, when indicated, attends doctor visits with mentor and/or individual receiving services.
  • Administers medications according to established guidelines. Provides documentation for all medications given and reviews medication administration records for compliance.
  • Participate in on call system as needed.
Qualifications

Requirements:

• Graduate of an accredited Registered Nursing (RN) program
• Experience working with adults with intellectual disabilities preferred
• Minimum 2 years of clinical experience

• Current RN license
• DHS RN Trainer status preferred; ability to become DHS RN Trainer required
• Knowledge of DHS CILA Rule 115 preferred
• Knowledge of DHS Medication Administration Rule 116 required
• Successful background clearance as required by state, licensing and company rules and regulations
• Valid driver's license and reliable transportation for on the job use
