Client Care Coordinator
 
The Client Care Coordinator (CCC) is primarily responsible for all client inquiries, Care Consultations, introduction of CAREGivers and Quality Assurance Visits.  The CCC is expected to assist and support Care Coordinators, RRC, CSR in order to provide the highest quality of care to clients.
Primary
  • Answer each call in a friendly, professional and knowledgeable manner
  • Develop and maintain relationships with clients and CAREGivers
  • Primary staff member for taking all inquiry calls and use the consultative approach to all service inquiries
  • Schedule and conduct Care Consultations
  • Email, write, call, etc … or follow up any way possible to all inquiries who have not signed up as a client
  • Input new client in BOSS with thorough notes for Care Coordinators
  • Inform Billing of any special circumstances and turn in deposit, credit card or EFT information
  • Conduct phone QA call after first shift, after first week and monthly thereafter, quarterly face-to-face.
  • Conduct telephone QA visits on all private clients every month
  • Conduct face-to-face QA visits on all private clients once each quarter
  • Counsel CAREGivers on quality concerns after visiting clients
  • Enter and maintain accurate client and CAREGiver records in BOSS
Secondary
  • Support other staff positions by assisting with functions such as collecting Client Notes, answering questions regarding possible employment, or taking messages as needed.
  • Participate and attend all quarterly CAREGiver meetings
  • Participate and attend all staff meetings
  • Attend International Convention at least every other year
  • Maintain neat and orderly working environment.
  • All other duties as assigned.
