This is a Clinical Manager role in the TRICARE Disease Management Programs. This individual will lead disease management programs in the following areas: heart failure, Diabetes, Asthma, COPD, Depression/Anxiety, and Cancer Screening.
*** This is an office position located at 2222 W Dunlap Ave, Phoenix, Arizona***
Primary Responsibilities:
- Responsible to complete the Disease Management program plan and lead development activities prior to operations
- Responsible for the successful delivery of the Disease Management programs as designed for TRICARE
- Once operational, lead Disease Management teams and manage the day to day operations; P&L; and overall program delivery, management, issue resolution
- Plays lead role in setting direction and participating in developing new programs
- Ensures team meets established performance metrics and performance guarantees
- Adapts departmental plans and priorities to address business and operational challenges.
- Influences or provides input to forecasting and planning activities.
Requirements
- Candidates must possess a current and unrestricted RN license
- 3 -5 years' experience in successfully managing disease management programs (people, processes, tools and budget) that include heart failure, diabetes, asthma, COPD, depression and anxiety
- Proven experience in development of Disease Management products, tools, and reports for these programs
- Proven experience in disease management - report development, analysis, and implementation of program improvement based on the data with improvement in metrics
- Advanced level of Computer skills with emphasis on MS Office suite of products i.e. Excel, Word, PowerPoint,
- Excellent communication skills with the ability to interrelate with individuals at various levels and in various functional areas throughout the organization
- Develop effective business relationships across business and product teams
- United States Citizenship
- Ability to obtain favorable adjudication following submission of Department of Defense
- BSN required and Master's degree preferred
- CCM certification preferred
- Prior start up experience preferred
- Prior military health plan experience preferred
