This is a Clinical Manager role in the TRICARE Disease Management Programs.  This individual will lead disease management programs in the following areas:  heart failure, Diabetes, Asthma, COPD, Depression/Anxiety, and Cancer Screening.

Primary Responsibilities: 




  • Responsible to complete the Disease Management program plan and lead development activities prior to operations 


  • Responsible for the successful delivery of the Disease Management programs as designed for TRICARE 


  • Once operational, lead Disease Management teams and manage the day to day operations; P&L; and overall program delivery, management, issue resolution 


  • Plays lead role in setting direction and participating in developing new programs 


  • Ensures team meets established performance metrics and performance guarantees 


  • Adapts departmental plans and priorities to address business and operational challenges. 


  • Influences or provides input to forecasting and planning activities.



 

Requirements

Required Qualifications:


  • Candidates must possess a current and unrestricted RN license 

  • 3 -5 years' experience in successfully managing disease management programs (people, processes, tools and budget) that include heart failure, diabetes, asthma, COPD, depression and anxiety

  • Proven experience in development of Disease Management products, tools, and reports for these programs

  • Proven experience in disease management - report development, analysis, and implementation of program improvement based on the data with improvement in metrics

  • Advanced level of Computer skills with emphasis on MS Office suite of products i.e. Excel, Word, PowerPoint,

  • Excellent communication skills with the ability to interrelate with individuals at various levels and in various functional areas throughout the organization

  • Develop effective business relationships across business and product teams

  • United States Citizenship

  • Ability to obtain favorable adjudication following submission of Department of Defense

Preferred Qualifications:


  • BSN required and Master's degree preferred

  • CCM certification preferred

  • Prior start up experience preferred

  • Prior military health plan experience preferred

As a requirement of UnitedHealth Group's contract with the Department of Defense, this position requires U.S. citizenship and proof of favorable adjudication following submission of Department of Defense eQuip Form SF86, (the National Agency Check Legal and Credit or NACLC).  Successful completion of the NACLC process is a requirement for continued employment in this role.  NACLC processing will be initiated by our TRICARE Security Officer post-offer, and can take 3-6 months for a final decision communication from the Department of Defense.  Candidates will be allowed to begin employment with UnitedHealth Group in this role based on an interim clearance, and final results will be communicated as they are received.  Failure to obtain final NACLC approval will result in termination from this role.
