Normal clinic environment. Ability to pass FIT testing and wear protective mask as deemed by SMA in the course of work at clinic.


  


This LPN position is responsible for direct/indirect patient care in the clinic which includes Medication Administration, rooming patients, handling phone calls and messages from patients, notifying patients of test results by telephone and mail, refilling prescriptions according to protocols, coordination of patient referrals, screening walk-in patients, assisting the providers and team members with procedures and interventions as needed and undertaking administrative duties as designated.

 

** Work Monday-Friday from 8:30 a.m- 5:30 p.m. at our The Siena Heights Clinic**

 

Primary Responsibilities:


  • Compliance with customer services standards, billing functions to include patient check in/out process (SMA 180-2), cash handling, scheduling and entering appointments, demographics, impanelment and insurance information in IDX

  • Assist with phone calls and messages. Determines best actions and collaborates with team to assign accountabilities to resolve

  • Responsible for collection and documentation of patient information to include vital signs, chief compliant, review of medications, allergies health risk screen information and other services

  • Ability to direct patients to the team member that would resolve issues prior to and after the Primary Care appointment. Participates in the monitoring of patients with chronic disease. Ensure patients meet post hospital visit requirements, medication refills and follow up appointments.

  • Functions as a member of the interdisciplinary team to facilitate care, assists RN with the coordination and completion of patient care plan

  • Responsible for compliance requirements such as laboratory QC management, waive testing, maintenance of crash cart and drug count, as well as other requirements dictated by clinic needs

  • Provides clinical intervention, such as drug administration, treatments, and procedures as ordered by provider and within scope of practice

  • Other duties as assigned to include, but not limited to, assisting coworkers, float to other areas and training/coaching of Medical Assistants

  • Follow standard protocols, processes, and policies to include but not limited to the following: Medication Refill, Preventative Services, Managing Conditions, and Disease Case Management as assigned by department head or designee 

Requirements

Required Qualifications:


  • Graduate from a accredited LPN program

  • Minimum two years of clinical experience

  • IV Certification preferred or ability to pass IV Competency skill test within six months of hire

  • Licensed Practical Nurse with active Nevada license

  • Current BCLS./ CPR card or able to obtain it in 30 days

  • Computer Literate (Electronic Medical Records, Microsoft Suite exp)
