Case Manager

This is a nonprofit healthcare organization serving northwest Ohio and southeast Michigan. Driven by their Mission to improve overall health and well-being, they offer a full range of diagnostic, medical and surgical specialties in areas such as emergency medicine and trauma, heart and vascular, oncology, orthopaedics, neurology, and other services.

*Will asses, plan, coordinate and monitor caseloads in Cincinnati
*Will work from home, local offices and from the road when visiting patients in home
*Collaborate with health care teams and patient families to ensure appropriate care
*Will assess HMO/Advantage members to determine case management needs
*Will plan a flexible care plan with member and health care team
*Coordinate current treatment plans and utilize cost benefit analysis
*Review patient progress and conduct reassessments
*Evaluate and manage resources to optimize reimbursement
*Communicate with patient and patient families
*Maintaining consistent documentation of patient progress, needs and treatment
