The CarePlus Personal Visiting Provider (PVP) is a Nurse Practitioner or Physician Assistant who is responsible, as part of the patient care team, for working with patients with chronic or unstable health conditions in order to prevent hospital admissions and/or readmissions, and/or to decrease inappropriate emergency department use.  An advocate for the patients, the PVP will work collaboratively with the patients' primary care provider, and with the CarePlus program's Medical Director and patient care team. The PVP will communicate and coordinate the patients' care with care providers, family, and caregivers. The PVP will provide timely, patient-centered, evidence-based care, intervening to treat acute or emerging problems as well as managing chronic conditions.



The PVP works to identify and develop innovative actions to meet the needs of CarePlus patients in both the health care and psychosocial / socioeconomic dimensions of care.  The PVP works with the CarePlus Care Management Team to located resources to address service gaps.



The PVP provides comprehensive initial and ongoing assessment, hospitalization trigger identification and management, chronic disease stabilization and monitoring, acute interventions within scope, advance care planning, documentation and patient education.

The PVP ensures that all pertinent and active medical conditions are documented in the medical record, and meets visit metrics in a manner compliant with CMS, Optum and client requirements.

Requirements

Required Qualifications:



  • Active un-encumbered NP or PA license to practice in the state of Michigan

  • Nurse Practitioner Certification from either the ANCC or AANP

  • DEA license or eligibility

  • Geriatric, Adult Medicine or Family Medicine specialty

  • Must be able to effectively communicate with elderly and chronically ill patients and families

  • Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial

  • Comfortable working independently in a home-visit setting

  • Basic knowledge of computers, electronic health records and cell phones

  • Familiarity with visit coding

  • BCLS

  • Excellent organizational skills with the ability to multi-task

 


Preferred Qualifications:



  • Minimum of 1 year Nurse Practitioner experience

  • Bi-lingual or multi-lingual a plus
