*** This is an office-based position located in our Ashford / Dunwoody office. ***


Social workers are responsible for providing social work services to patients and families. They function as a member of the departmental team which includes educating the patient, family and members of the healthcare team regarding benefits, community resources, referrals for counseling and other pertinent information. Social workers are also responsible for triaging referrals and collaborating on cases with other members of the healthcare delivery team.

 


Primary responsibilities:


  • Assures that patients and families are knowledgeable of available community resources and how to access them

  • Serves as a resource to clinical staff in dealing with complex psychosocial issues and accessing community resources

  • When appropriate, provides direct Care Coordination services to patients with complex social/emotional needs

  • Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical record

Requirements


Requirements:


  • A current, unrestricted Georgia mental health license (LCSW, LPC, PhD) or RN with behavioral health experience

  • Must be a Master's degree-level clinician

  • 2+ years of post graduate social work experience

  • Computer proficiency  - Internet, Word, Email

  • Strong Collaboration skills

  • Ability to work in a fast-paced environment

 

Assets:


  • Case management experience

  • Knowledge of community programs/services

  • Call center/telephonic experience highly preferred

  • Experience in discharge planning

  • Experience working in managed care

  • Experience in Inpatient facility
