Primary Responsibilities:



  • Create a positive experience and
    relationship with the member.
  • Keep the member out of the
    hospital by facilitating regular visits to their primary physician.
  • Encouraging member to be actively
    engaged with their primary physician.
  • Proactively engage the member to
    manage their care.
  • Provide member education.
  • Support transitions of care.
  • Help to keep members compliant
    with their preventive healthcare needs.
  • Partner with care team
    (community, providers, internal staff).
  • Knowledge and continued learning
    of community cultures and values.
  • May conduct HRA assessments if needed.
  • Transferring
    members as appropriate to clinical staff if clinical need identified.


Requirements



Required
Qualifications:



  • High School Diploma/GED.
  • 2+ years of Telephonic Customer Service experience.
  • Must be able to create,
    edit, save and send documents utilizing Microsoft Word and Excel.
  • Must be able to quickly
    learn new software programs.
  • Must be able to talk &
    type at the same time.


 



Preferred
Qualifications:



  • Bilingual in Spanish, Russian or Chinese.
  • Previous Telecommute experience.
  • Experience working within the Healthcare Industry.
  • Experience working with Healthcare Insurance.
  • Experience working in a Hospital, Physician's Office, or
    Medical Clinical setting.
  • Professional experience in a clerical or administrative
    support related role.
  • Working Knowledge of Medical Terminology to communicate
    with members and providers.
  • Experience working with Medicare and/or Medicaid Services.
  • Experience working in a Call Center environment.
