CNA / HHA / Caregiver (PT & FT) As a designated care provider at Sunrise, you will be at the center of what we do best, caring for the residents of our communities within the Reminiscence neighborhood. Requirements: You will build meaningful relationships with a specified number of seniors and their families as you provide assistance with activities of daily living, attend to individual care needs, and get to know their unique preferences and personalities.You will notify management of changes in condition and recommend adjustments in the level of care and service.You will assist residents in life skills and other life enriching activities as indicated on their individual profile.You will blend a variety of multi-sensory experiences into the residents' day. Life Enrichment Manager (PT) The Life Enrichment Manager is responsible for consistently engaging residents in familiar routines of daily living through personalized Life Skills that "Create Pleasant Days" for residents. The Life Enrichment Manager is responsible for designing Life Skills to reflect each resident's past interests and for providing them with encouragement, prompts and hands on assistance. Requirements: One (1) year experience working with memory impaired seniors. High School diploma / GED accepted; certification(s) may be required per state regulations. Knowledge of how to adapt life skills to the cognitive and functional ability of each resident. Inspire, motivate and encourage volunteers and fellow team members. Ability to work semi-independently without direct supervision. Basic proficiency in computer skills, Microsoft Office with the ability to learn new applications. Medication Care Manager (PT) In this key role you will be responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe and organized manner, and documenting medication administration and resident care. Requirements: Medication Management certified. Minimum of six (6) months experience working with seniors in assisted living, home health, independent living, hospital or long term care environment and desire to serve and care for seniors. You must be able to make choices, decisions and act in the resident's best interest, have the ability to react and remain calm in difficult situations and possess written and verbal skills for effective communication and level of understanding. LPN Medication Care Manager (PT) The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Requirements: L.P.N, L.V.N. or state specific licensed nurse credential. Minimum of six (6) months experience working with seniors in assisted living, home health, independent living, hospital or long term care environment and desire to serve and care for seniors. You must be able to make choices, decisions and act in the resident's best interest, have the ability to react and remain calm in difficult situations and possess written and verbal skills for effective communication and level of understanding. Wellness Nurse (PT) The Wellness Nurse is responsible for monitoring the health and well-being of our residents and providing leadership support and assistance with resident care; assisting in training and monitoring of medication care administration, communicating with resident support services and families, and maintaining quality assurance. Requirements: Must maintain a current state licensure as a professional Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN). At least one (1) year of experiencein home health, assisted living, hospital or long term care environment. Demonstrated knowledge of nursing practices, techniques and methods applied to health and wellness resident care. Medication administration. Ability to manage multiple priorities.
