Pennsylvania Mentor is seeking an Intensive Case Manager four our office in Bethlehem, Pennsylvania. This position will require extensive travel throughout the Bethlehem/Allentown area.

To that end, the Intensive Case Manager role is a fantastic role to continue your career path in Social and Human Services with a myriad of career options to move up and across the organization. Community based case management services are provided to children and adults residing in Northampton and Lehigh Counties.
Key Responsibilities:
- Facilitation in the area of Life Skills.
- Assisting with setting up community services to increase client independence.
- General case management.
- Crisis Intervention and Prevention.
- On Call Rotation required.
Requirements:
- Bachelor's Degree in counseling, social work, or related field, from an accredited college or university preferred.
- At least (1) year's professional experience in the provision of casework services or service coordination to children and families involved with the child welfare/juvenile justice/education systems.
- Ability to maintain effective working relationships with supervisors, peers, students, families and outside agencies.
- Demonstrated ability to work effectively with emotionally handicapped adolescents and their natural and/or foster families
- Must have valid driver's license and a reliable transportation.
- Extensive travel throughout the Lehigh Valley area is required.
