REM South Central Services
REM South Central Services, a part of REM Minnesota, supports individuals with intellectual and developmental disabilities, as well as autism spectrum disorders, mental health challenges and other complex needs. In a variety of home and community-based program settings, REM South Central Services offers residential services with supports tailored to the needs of each individual as well as hourly support, respite and other specialized services to help individuals be as independent as possible.
DescriptionREM South Central Services is seeking a Program Coordinator to provide assistance in a community-based setting. They primarily work Direct Care with the individuals served that includes: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. They may also facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
Other Duties performedby Program Coordinators include, but not limited to: facilitate theimplementation of Coordinated Service and Support Plan (CSSP) and participatein a range of administrative duties including on-site interviews, teammeetings, corporate meetings, training, scheduling and staffing oversight.
QualificationsRequirements:
- High school diploma or GED
- One year related work experience
- Must be 18 years of age
- Valid driver’s license in good standing
- Successful clearance of local and/or state background checks
Preferred Skills:
- Supervisory experience
- Background with Mental Illness/ Dual diagnosis
