The Provider Services Onsite Case Management Clinical Manager is responsible for the overall performance of the clinical program he/she supports. The Clinical Manager develops and leads professional clinical and nonclinical staff in required activities which facilitate and support quality, cost efficient outcomes and minimizes fragmentation of health care delivery.


 


The Clinical Manager has experience and expertise in the Case Management process and coaches the staff on the collaborative process of assessment, planning, implementation, coordination, monitoring, and evaluation to address patients holistically. The Clinical Manager supports and coordinates the operations effectiveness across the continuum of onsite and telephonic case management.


 


The Clinical Manager helps his/her staff to understand and demonstrate the ability to integrate the needs of consumers, customers and the business. Overall, the Clinical Manager is held accountable for the team's ability to meet the business goals and will continually motivate the staff to develop the skills needed to succeed in their roles.


 


The Clinical Manager will meet with hospital representatives, physician practices and other members of the care system to develop relationships which facilitate communication and coordination amongst the health care team to minimize fragmentation of services.

  • Meets business goals and objectives

  • Meets and/or exceeds customer expectations

  • Collaborates with the provider services OH team, physicians, facilities and community resources

  • Collaborates with sales, account management, product organization, and other stakeholders as applicable

  • Responsible for performance management of staff

  • Articulates team performance and impact of services delivered during client meetings

Requirements

Requirements:


  • Current, unrestricted RN license in the State

  • Minimum of 3+ years' experience as a manager and/or supervisory

  • 3+ years' experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company

  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment

Additional Assets Preferred:


  • BSN

  • A background in managed care

  • Case Management experience

  • Certified Case Manager (CCM)

  • Experience/exposure with discharge planning

  • Experience in utilization review, concurrent review and/or risk management

  • Strong organizational skills and multitasking abilities will be keys to success
