Key responsibilities include:
* Completes functions related to physician signed documentation processing and tracking for Home Health patients.
* Responsibilities include, but are not limited to faxing information to physicians, sorting, scanning and attaching physician signed documentation into Electronic Medical Records (EMR) system.
* Responsible for data entry associated with Home Health episodes for tracking documentation, orders sent, and orders received with signatures, and ensuring accuracy of data entry in accordance with daily and weekly reconciliation procedures.
* Reports daily any items outside of agency established timely parameters and coordinates with Clinical Supervisor and care team staff to comply with agency timeline standards.
* Ensures orders, and episode discharge summary (at discharge, re-certification, and transfer) are sent to physicians timely, according to agency standards.

We seek the following principal qualifications:
*
High School Diploma required. Associate’s or Bachelor’s degree preferred.
* 1 year of experience in a related health care office setting. One year home health office experience preferred.
* Proficiency with computer use and software applications including Microsoft Office and EMR software preferred.
* Must have exceptional customer service and organizational skills
* Ability to work varying hours, including weekends, nights and holidays.
