HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

The position leads a team of case managers responsible for accurately assessing, negotiating levels/rates and managing stay of managedcare patients admitted to HCR ManorCare centers. Responsible for facilitating interdisciplinary plans and assuring progress reports are completed and provided to payor as required; Also serves as liaison between patient, physician, care teammembers, payor, and the discharge planner by coordinating, monitoring, and communicating patient's progress and cost evaluation and assisting with coordination to the next level of care.

The Manager of Case Management will be responsible for coverage throughout New Jersey, Delaware, Virginia, the Carolinas, Georgia Florida and Texas.  Prefer candidates that reside within 45 minutes of an airport as travel throughout the division is required.

Educational Requirements:Certified Case Manager (CCM) or Certified Insurance Rehabilitation Specialist (CIRS). Bachelor's degree in nursing preferred; therapy or health care administration degree considered. Currently licensed to practice profession in the state.

Position Requirements:

Three to five years of clinical practice experience or certification in case management with two years managed care experience at a professional level. Demonstrate evidence of continuing education to maintain clinical expertise and retain or work toward Case Manager certification. Ability to effectively communicate and interact with patients and practitioners. Effective negotiation skills. Ability to assess patient's readiness for change and demonstrate sensitivity to culturally diverse situations. Proven time management and organizational skills and strong research and analytical skills. Ability to work in a fast-paced, matrix driven clinical environment. Preference for managed care experience and computer knowledge.
