The Manager, Diabetes Case Management will be responsible for managing a Diabetes Case Management team located in Greensboro, NC.

Primary Responsibilities:

• Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)


• Case management and coordination of care


• Takes lead role in setting direction and participating in or developing new programs 


• Sets team direction, resolves problems and provides guidance to members of the team


• Ensures team meets established performance metrics and performance guarantees


• Adapts departmental plans and priorities to address business and operational challenges

Required Qualifications:

• 2+ years of experience as a manager


• 3+ years experience with Diabetes management
• 3+ years of experience in a clinical setting
  
• Proficient level of experience with Microsoft Word, Excel, PowerPoint, and the ability to navigate a Windows environment


• Ability to perform in a fast paced, high demand environment


• Adapts well to frequent change in process


• Must be able to talk and type at the same time


• Must live within a commutable distance of Greensboro, NC office (50 min or 50 miles)

Preferred Qualifications:

• Bachelor's Degree in Nursing


• Clinical Diabetes treatment experience


• Previous Case Management or other managed care experience


• Certified Case Manager (CCM)


• Experience working with the Medicare guidelines and regulations


• Call Center experience
