This Job Description for the Staff RN on the Transitional Care Area provides criteria for evaluation of the Registered Nurse working in the Staff RN position. The Staff RN assigned to the Transitional Care Area cares for adult and geriatric patients.

POSITION RESPONSIBILITY

E1. Responsible for assessing, planning, implementing and evaluating care for assigned patients.

E2. Responsible for meeting the Standards of Practice for the Transitional Care Area and managing all assigned personnel, supplies, and equipment.

E3. Participates in activities to plan, measure, asses and improve the quality of patient care.

E4. Responds to Code Blue and implements life-saving measures.

E5. Assesses patients in an on going manner, per protocol.

E6. Assists physician with procedures, as needed (venous access devices).

E7. Responsible for assessment of patient with telemetry monitoring.

E8. Responsible for maintaining customer friendly atmosphere.

E9. Draws blood from venous lines.

E10. Models and reinforces behavior consistent with goals for teaching the patient, communicates patient status to appropriate member of the healthcare team, and provides support to the family, as needed.

E11. Scans supplies from P.O.U. area.

E12. Performs and documents admission assessments

E13. Starts and/or maintains existing IV lines and starts first does antibiotics or STAT meds ordered

E14. Facilitates placement of patients on inpatient units in an expedient fashion. Communicates condition of patient to receiving unit after patient is discharged from the Direct Admit Center.

POSITION REQUIREMENTS

Experience:

1. One year as staff nurse preferred.

2. One year or more of charge nurse experience preferred.

3. Telemetry unit/ ICU/ ER experience preferred.

Education:

1. Graduate of accredited school of nursing

2. Educational requirements appropriate for meeting age-specific needs of patients assigned to the Transitional Care Area.

a. Nutrition, i.e., alteration in nutrition, less body requirements.

b. Hazards of immobility, i.e., impaired physical mobility.

c. Impaired verbal communication or special communication needs.

d. Grieving process.

e. Alteration in elimination.

f. Self-care deficit.

g. Sleep pattern disturbance.

h. Impairment of skin integrity

i. Activity intolerance

3. Educational requirements appropriate for meeting age-specific needs of adolescents, including:

a. Knowledge deficit.

b. Anxiety related to hospitalization.

c. Alteration in nutrition, less than or greater than body requirements.

d. Potential for injury.

e. Powerlessness.

f. Coping, ineffective individual or family.

g. Disturbance in self-concept.

h. Non-compliance.

Special Qualifications

1. Current Texas licensure with State Board of Nurse Examiners

2. Current BCLS certification from the American Heart Association required.

3. Demonstrated current clinical competence, as outlined in the appropriate orientation checklist.

4. Medication module test score of 80% or remediation.

5. Maintains current clinical competence
