BMT Transplant Coordinator

The Bone Marrow Transplant (BMT) Unit is an acute care unit that provides care for adult and geriatric patients with medical oncology, benign hematologic, and or hematologic oncology diagnoses requiring treatment with blood and marrow transplantation. The Emergency Department, PACU, Physicians' Offices, Critical Care Units and Patient Registration are all entry points for patients admitted.

Primary Services:

Chemotherapy/Biotherapy administration and management, including investigational protocols

Transfusion of blood, platelets and fresh frozen plasma.

Management of central venous access devices.

Management of spinal drains.

Bone marrow biopsy and aspiration.

Thoracentesis and paracentesis.

Epidural and intrathecal analgesia.

Management of toxicities r/t chemotherapy/biotherapy/radiation therapy.

Management of neutropenia/thrombocytopenia/anemia

Antibiotic/Medication therapies.

Pain management.

Blood and Marrow Transplant, autologous and or allogeneic (related & unrelated donor).

Bone marrow harvest recovery.

Telemetry monitoring.

Centennial Medical Center is currently looking for a BMT Transplant Coordinator.

Responsibilities:

As an expert clinician, is responsible for coordination / facilitation of adult complex hematologic malignancy and stem cell transplant patient care and services across inpatient and outpatient settings from first consult through long term follow-up. S/he collaborates with the multi-disciplinary team to assure an organized approach to delivery of care. S/he acts as a liaison with physicians, internal / external departments and other agencies related to creation and follow through of the patient’s individualized treatment plan. S/he communicates with responsible parties about any scheduled or unplanned deviations from the plan of care and works to resolve /remove obstacles to access or delivery of recommended therapies.

1. Monitors all phases of cancer related services including, prevention/screening, diagnosis, treatment, survivorship, palliative and or end of life care for patients and caregivers

2. Acts as a liaison/ advocate for patient, donor, and caregivers as they navigate health care systems

3. Directs scheduling of / schedules screening and diagnostic tests, procedures and provider appointments for patients and or donors.

4. Creates and communicates patient and or donor treatment calendars clearly outlining plan including initial and updates as care plan changes

5. Routinely contacts patients, donors, caregivers to check on health status and or service needs

6. Provides education to patients, donors, and caregivers

7. Collaborates with financial coordinators to ensure verification of third-party payer coverage; assists with financial coordination of treatment/ transplant processes

8. Works with NMDP / cooperative registries to identify and select unrelated donors; coordinating all steps of unrelated donor processes

9. Completes and submits appropriate NMDP forms by defined timelines

10. Coordinates weekly intake (patient selection) meeting; maintains up to date patient list on share drive

11. Attends IP and OP multidisciplinary rounds, regularly, serving as an expert clinical resource to inpatient and outpatient physicians and or staff

12. Develops and maintains patient care forms such as order sets, practice guidelines, and consent forms.

13. Develops, reviews, revises patient, donor, and caregiver education materials

14. Assists in the development of and adherence to regulatory standards, participates in regulatory /COE surveys and site visits.

15. Participates in PI activities

16. Performs internal audits of patient charts, TED forms, etc as requested based on annual QMP calendar

17. Assists with completion of Clinical and COE RFIs (requests for information)

18. Accurately enters data into defined database/ shared drive fields within designated timelines

19. Participates in activities that promote professional growth

20. Participates in marketing/community service events that promote the organization/program

• Valid TN RN license

• 3 years oncology experience including stem cell transplant and or heme-malignancy,

• adult setting preferred

• Current AHA BLS,

• ONS Chemotherapy Certification required within six months of hire

• BMTCN or CHTC certification required within 4 years of hire (BMTCN is not available until 2.2014)

• BSN or Masters degree,preferred

• Apheresis experience, preferred
