About this job

Under the direction of the Program Director is responsible for the day-to-day direction of social service patient care. This includes quality care of the patient from admission to discharge. Establishes and maintains effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments. Considers ages of patients when fulfilling patient care responsibilities. Demonstrates the ability to obtain and interpret information in terms of the patient's needs; possesses knowledge of growth and devlopment; and has an understanding of the range of treatment needed for these patient groups. Works with the Director and Medical Director as Performance Improvement Coordinator, oversees and complies results of performance improvement studies.

Accurately completes documentation on patient record.

Acts as advocate for patient safety.

Acts as mentor and preceptor for new social service employees.

Assesses patient/family learning needs, abilities, preferences and readiness to learn.

Collaborates with the Program Director in the Maintenance of the program milieu.

Coordinates with UR to ensure fiscal accountability for services rendered.

Demonstrates ability to function as a member/leader of interdisciplinary care delivery team by assessing, collaborating and coordinating care and discharge planning needs.

Ensures effective care and use of time, supplies, and equipment.

Fosters and obtains a commitment for the multidisciplinary team concept on the units.

In collaboration with the patient and family, makes assessments and recommendations. Participates in the development of patient plan of care to include patient problems, goals evaluations and interventions.

Incoporates cultural and developmental needs into plan of care.

Maintains awareness of age-appropriate resources for patient support. Identifies appropriate community resources for follow-up care, refers as appropriate.

Participates in in-services, staff meetings or training programs.

Provides for professional growth and development of social service staff through identification of needs, participation in training programs, encouragement of formal continuing education efforts, and evaluation of same.

Provides oversight of involuntary commitment process perphysician order and coordinates such with court authorities as necessary.

Recognizes and responds to differences in legal consent issues and release of information for patient and families.

Reflects a positive, professional image to others.

Verbalizes/demonstrates knowledge of the process for resolution of ethical dilemmas. Accesses ethics committee as appropriate.

Licensure/ Certification/ Registration: LCSW or CSW license required; Healthcare Provider BLS certification. Completes necessary direct service and supervision hours required for obtaining and/or maintaining LCSW or equivalent licensure.

Education: MSW or equivalent.

Experience: 2 years clinical experience and one year management experience

preferred.

Special Qualifications: Evidence of leadership skills, sucessful interpersonal realtions demonstrated by active participation in problem solving and accepting increased responsibility in supervision, administration and education. Excellent customer service skills.
