Trinity Community is seeking an experienced, compassionate MDS Nurse to enhance the quality of life of our residents in all areas.
Position Summary:
The MDS Nurse is responsible for coordinating the care delivery interventions for residents in the assigned area in accordance with current professional practice standards, physician's orders, facility policies and procedures and local, state and federal regulations and within the scope of practice.
Position Highlights:
* Asses, plan, implement and evaluate services and education to be delivered to the family in coping with and participating in the management of the resident's care
* Participates in assessing, planning, implementing and evaluating resident care delivery with actual outcome
* Assists in completing the initial and ongoing nursing assessments needed to plan care and interventions
* Assists in documenting and observing changes in condition and unusual incidents and reports findings to supervisor, physician and responsible party, as required
* Completes the care plans and is involved in care conferences
* Completes all forms used throughout the residents' charts according to policies and procedures
Requirements:
* Minimum of 3 to 5 years' experience as MDS Nurse
* Current LPN or RN license in state of Ohio (RN preferred)
* Experience with Point Click Care, Trendtracker and Microsoft Office Suite preferred
* On-call required
