POSITION SUMMARY
Work based from home, facilitate client participation in treatment plans towards full rehabilitation including regular contact with client and providers; as well as coordination and attend appointments with client and providers. Problem solve clinical issues and document progress
Fundamental Components

- Experience problem solving clinical issues
- Organized, able to coordinate schedule
- Have a reliable car and be willing to travel 100 - 150 miles per day, 5 days per week
- Have a secure workspace at home to work uninterrupted
- Able to work early morning or late evenings as needed
- Strong technology skills including keyboarding, use of MS Office, and smartphones
50% of time spent traveling and attending appointments with clients and medical providers to review medical status and ensure provider is following clinical protocols per the diagnosis. 25% of time spent arranging, facilitating and confirming client participation in appropriate treatment plan. 25% of time spent documenting activities and making recommendations
BACKGROUND/EXPERIENCE desired
Must have valid State of MI Registered Nurse license and be in good standing.
- Certified Case Manager preferred but not required.
- Field Case Management experience or Home Health Care Experience preferred.
- Experience with Customer Service
EDUCATION
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience
