The Medical Social Worker provides counseling and supportive services to the patient, their family, caregivers, and SCAH staff. This position assesses the social and emotional factors related to the patient’s illness and evaluates the psychosocial response to treatment and level of adjustment. This position interprets to the family, patient, physician, and staff, the social and emotional factors related to illness, financial and community resources. Education Masters degree from a program accredited by the Council on Social Work Education. Must possess current BCLS CPR certification. Experience One year of experience as an MSW in a health care setting is strongly preferred. Home Health experience preferred. Knowledge Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues. Knowledge of home care setting and regulatory guidelines. Knowledge of local community resources is required. Knowledge and understanding of individual development and human behavior as it relates to the effects of illness; and of the influence of culture on health care. Knowledge of state and federal homecare or hospice regulatory guidelines preferred. Skills Demonstrated skills in effective written and verbal communication. Ability to recognize the needs and concerns of diverse groups of people. Must be sensitive to situations encountered in the provision of hospice care. Ability to maintain harmonious constructive working relationships with internal and external customers including patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources. Must have well developed social work assessment and clinical skills as well as time management skills. Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter Care at Home’s strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor. Must be able to deal with challenging work environment with time demands and occasional conflicting priorities. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements. Is responsible for maintaining all required licensure and certifications. At orientation and annually thereafter, must be able to meet Sutter Care at Home core competencies for position and area of specialty, as appropriate. Must possess a current California driver’s license and access to a reliable automobile with proof of liability insurance.
