Sutter Care at Home is the home health care and hospice affiliate of Sutter Health, one of the nation's largest not-for-profit health care systems.

Under the general direction of the Director of Patient Care Services/ Designee, the Licensed Vocational Nurse assists in the nursing process by instructing patients and care givers in self care and providing basic nursing care as needed. The LVN may also provide continuous care, under the direction of the Case Manager/designee, provides skilled nursing care to terminally ill hospice patients in accordance with the treatment and plan of care dictated by patient, family, physician, and members of the interdisciplinary team. Continuous care is defined as a minimum of 8 hours of care within a 24 hour period with at least 51% of the care provided by a skilled, licensed nurse. Education Graduate from an accredited vocational school of nursing or equivalent. Current California LVN license and Blood Withdrawal certification. Must possess current BCLS CPR certification. Experience Minimum 1 year prior experience in an acute care setting or equivalent experience is strongly preferred. Home health or community health experience preferred. Knowledge Basic competencies in the fundamentals of nursing and pharmacology are required. Must have a flexible schedule and be willing to be called onto a continuous care shift with little or no notice. Familiarity with interpersonal dynamics operative within a health care team and knowledge of the dying and bereavement process. Knowledge of hospice principles and practices within the scope of hospice and palliative care and/or end of life issues. Excellent observation, verbal and written communication skills, problem-solving skills, critical thinking skills, nursing skills per competency checklist. Special Skills/Equipment Demonstrated skills in effective written and verbal communication. Ability to recognize the needs and concerns of diverse groups of people. Ability to maintain harmonious constructive working relationships with internal and external customers (patients, families, members of the interdisciplinary team, clerical and management staff, physicians and community resources). Ability to use and give instruction of adaptive equipment. Must possess a current California drivers license and access to a reliable automobile with proof of liability insurance.
