As a Field-Based Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Primary Responsibilities:

• Nursing home chart reviews for coding and medication reconciliation


• Diagnosis reviews for coding accuracy


• Transcribe on call logs into electronic medical record


• Provide in services to partnering nursing home staff for development and new home openings


• Obtain vaccination reports from nursing homes and enter information into patient electronic medical record


• Monitor use of level 2 bed support surfaces to meet medicare criteria.


• Must travel to nursing home facilities daily throughout the Hartford, CT area

Required Qualifications:

• Registered Nurse (RN) with active and unrestricted license in CT  


• Bachelor's Degree (BSN)


• 5 years clinical nursing experience


• Basic Microsoft Office skills with the ability to navigate in a Window's environment


• Competency with electronic medical records.


• Driver's license, reliable means of transportation and appropriate insurance coverage

Preferred Qualifications:

• Homecare background


• ICD-9 coding experience
