The Acute Clinical Coordinator provides professional nursing care and coordination of services provided in various licensed acute hospitals and sub-acute facilities that have contracted with DaVita to provide dialysis and other services to patients of and under the license of the acute facility. Responsibilities include acting as team leader, supervising and administering direct patient care, communicating with the Acute Program Office to coordinate and organize assignments as well as patient treatment plans. This position also ensures documentation of all delivered care is complete and reviews essential quality assurance markers of care delivered with Facility Administrator for reporting.

Details: This is a full-time, variable shift position. The Acute Clinical Coordinator will be responsible for patient care, scheduling, supervision and helping drive clinical outcomes. Dialysis experience is required.

Current RN license within state of practice
Minimum of 6 months of current nephrology nursing experience required
Current basic EKG interpretation certification required
Current CPR certification required
Clinical supervisory experience preferred
CNN or CDN desirable
ICU, CCU, ER or Med Surg experience preferred
