I. TREATMENT PLANNING AND DISCHARGE PLANNING

Conduct Psychiatric Diagnostic Assessments, both initial and annual

Provide medication management services to assigned caseload

Evaluate the effectiveness, use, side effects of prescribed medications

Conduct required prior authorizations with the PA Call Center

Address medication concerns and respond in a timely manner

Perform required vital sign/weight/height measurements

Process medication refill requests

Administer medication if needed

Review Psychological Testing results with families/clients, when needed

On assigned therapy cases, is able to formulate clearly defined goals and objectives as evidenced by writing goals that

reflect the patient's presenting problems as seen in the assessments. Is able to accurately reflect progress of the patient specific to the treatment goals.

APN begins discharge planning at admission as seen on the intake assessment and treatment plan.

Work with a physician or alone in and outpatient clinic setting

II. ASSESSMENT OF PATIENTS

Demonstrates knowledge of developmental tasks of children, adolescents and adults.

Demonstrates the ability to recognize signs and symptoms of anxiety and impending behavioral escalation.

Ability to recognize and evaluate behavior, changing levels of depression, tendencies toward self-aggressive behavior, cognitive functioning deficits, learning deficits, and patient's capacity for self-care.

Exhibits proficiency in diagnosing and determining appropriate level of care based on client symptoms.

Identifies signs of abuse and neglect and follow necessary procedures to ensure client safety.

III. TREATMENT TEAM

Accurate information is communicated to all members of the team as observed by the supervisor

Works cooperatively and positively with all treatment team members as observed by the supervisor.

Is flexible and creative with job requirements and expectation as observed by supervisor.

IV. TREAMENT DELIVERY

Individual, family, and group therapy occur as prescribed by the physician approved treatment plan as evidenced by documentation in the medical record.

Demonstrates a willingness to be able to provide client-oriented services when needed, regardless of hours and times.

Ensures that parents/guardian/significant others are made aware of all assessments, evaluations and treatment results as assessed by the supervisor.

Achieve productivity expectations through the completion of required clinical hours.

V. DOCUMENTATION

Therapy notes reflect an awareness of the patient's pathology and problems, notes are goal directed and follow the prescribed format (P.I.E.) as evidenced by the notes in the medical record.

Assessments, treatment plans, and progress notes are completed timely according to Outreach guidelines as evidenced by submission to UR Specialist.

Provides accurate and complete documentation as observed by the UR Specialist.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Minimum of a Masters Degree in a nursing and have a license as a registered nurse. A minimum of two years psychiatric treatment experience is preferred along with experience in individual, group, and family therapy. Knowledge of regulatory requirements and Arkansas laws governing services to children, adolescent and adults preferred. A basic knowledge of educational, psychological and social service systems as related to special education and behavior treatment preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Appropriate State Licensure. Licensure must be maintained.
