Job Specific Details:

Assisting the Wellness Director/Resident Services Coordinator/Executive Director with delivering Service Plans. Documenting exceptions to Service Plans utilizing 24-Hour Reports and Individual Service Notes. Participating in weekly service planning sessions, as needed. Providing and assisting residents with services delineated in residents' individual Service Plans including:
personal care. Monitoring and communicating residents' changes in condition and service requirements to Supervisor. Following proper procedures for accounting for use of resident care supplies. Complies with monitoring and documenting mandatory hourly/courtyard checks.
Ensuring that residents are free from abuse, mistreatment, and neglect, and reporting any such instance, as
appropriate. Performing multiple tasks across functional areas and filling in for other staff when needed. Ensuring timely arrival of residents at scheduled programs and other appointments. Assisting and delivering daily programs and services. Assisting residents with transfer and meal service as indicated on resident Service Plan. Complying with food service and personal hygiene standards. Communicating with residents, families, and guests in a courteous manner; ensuring that all resident requests are responded to during meal service in a timely manner
